Preparing for Your Appointment

To help ensure your upcoming appointment goes as smoothly and efficiently as possible, please come prepared with the following:

  • A list of any questions you would like to discuss with your healthcare provider

  • A written list of your current medications, including over-the-counter medications and supplements (or bring the medication bottles with you)

  • Your insurance information and insurance cards

  • If you are enrolled in a HMO or managed care plan, please bring a referral for your visit

  • If you have been referred for diagnostic testing, please ensure your physician has provided the appropriate prescription or order for the test

  • If you are seeking a second opinion or a pre-operative evaluation, please bring any relevant medical records or test results that may assist in your evaluation (such as catheterization images, stress tests, echocardiograms, etc.)

 

To help streamline your visit, we also ask that new patients complete the New Patient Information Form prior to their appointment.

BILLING

​Goldman Cardiology Group participates with most major insurance companies. As patients are frequently changing insurance plans, it is important that we have your current and accurate insurance information at each visit. Please note: if your insurance coverage is provided by an HMO or managed care plan, you are required to obtain written referrals/authorization from your primary care doctor to be seen. Our staff is happy to help with any billing questions you may have. 

As a patient, you are responsible for all co-payments and deductibles specified by your insurance plan. These payments are expected at the time of service. For your convenience, our office accepts cash, personal check and most major credit cards.
